How to Become a Patient

Medical or Dental Care
To receive care individuals must make an appointment with a patient services volunteer to assess eligibility prior to seeing a doctor. Eligibility interviews are conducted by appointment only and can be made by calling the Clinic's main number at (281) 364-7889 during normal business hours.

You must bring identification; proof of income and proof of address to your eligibility interview (see below for further details).

Examples of Required Information:

1. IDENTIFICATION

  • Texas Driver’s License
  • Texas Identification card
  • Passport or Visa
  • ID from other country

2. PROOF OF INCOME 

  • 60 days of recent paycheck stubs
  • If paid in cash, individuals must provide a notarized statement from his/her employer indicating amount earned for last 60 days
  • If an individual’s support is provided by another person, that person’s income will need to be verified and a written statement provided stating that they are responsible for all the patient’s needs

3. PROOF OF ADDRESS

  • You must provide documentation showing where you have lived for the past month (example: utility bill, lease agreement, cell phone bill, insurance card, etc.)
